The Stroke Project
Educating – Supporting - Preventing
The Stroke Project is a Hackney based service that provides long
term, person centred support to stroke survivors’ and their
families. Finding yourself alone with a disability after community
rehabilitation services is a frightening time. We are on hand to
alleviate some of these fears. Viewing recovery in the long term, we
have the privilege of witnessing our members progressing towards
achieving personal aims, great or small. We also work within the
community to raise awareness of stroke and help people identify and
reduce their risk.
Long Term, Person Centred Support
The Stroke Project supports and empowers people affected by stroke
to ensure everyone has the opportunity to work towards achieving
their personal aims.
Stroke Survivors Groups - Space and
time in a supported environment to explore stroke with other stroke
survivors. Stroke focused physical, social, “brain stimulating” and
fun activities.
Community Stroke Outreach - Person
centred individual support for stroke survivors, families and carers
towards independent living. A link between community services,
health and social care, along the stroke pathway.
Communications Service - Community
group support for stroke survivors with communication difficulties
and workshops for community groups.
Information, Advice, Support – Regular
services for anyone who has been affected by stroke and needs
information or support to access local community services.

Stroke Prevention
Health Promotion -
Stroke awareness & prevention sessions
for community groups, care professionals. What is a stroke? Identify
symptoms of stroke, major risk factors, support to prevent strokes
and awareness of the effects of stroke.
Hypertension Project – Raising
awareness on hypertension and measuring blood pressure in the
community.
Vascular risk assessment – Supporting
the national initiative to help men and women identify and reduce
their risk of heart attack, stroke, diabetes and kidney disease.
Education
Dementia Training -Training workshop
for frontline health administration professionals to understand the
signs and symptoms of people affected with dementia.
Volunteers - Gain experience in health
and social care whilst supporting stroke survivors to regain
autonomy or promoting health.
Improving Community Stroke Services
Stroke Project Development Work
The Stroke Project is developing services across other boroughs in
London to help provide community based support services for people
affected by stroke their families and carers. The development work
is being carried out by ‘The Stroke Project’ who are working closely
with the North East London Stroke Network, Healthcare for London and
other stakeholders to provide long term care for ‘Life after
stroke’.
Joint Working - Collaborating with
local health, social care and voluntary sector partners to ensure
quality of services for people affected by stroke and raise
awareness of stroke.
Research Projects
Action research and consultations on local needs and best practice
of delivering stroke services in the community.
