Stroke Survivors Project
A stroke leaves every individual with a very personal battle.
The challenges are as unique as the person who has suffered the
stroke. The stroke survivors project works together with stroke
survivors to ensure everyone has the opportunity to work towards
achieving their personal aims however great or small. Committed
to understanding the spectrum of complications a stroke can bring,
the project supports survivors whatever their ability.
The project believes that ongoing care and support
is crucial if stroke survivors are to be given the chance of
the best possible recovery. Finding yourself alone with a disability
after primary care based community rehabilitation services have
finished is a frightening time, and the project is on hand to
alleviate some of these fears. Viewing recovery as a continuum,
the project has the privilege of seeing people make progress
long after it is assumed that their chance of further recovery
has passed
By recognising the individual as well as the stroke, the project
works to ensure the stroke survivor has all the necessary tools
to give them the greatest chance of leading an independent life.
These tools could be linking an individual with other services,
advocating on their behalf, or creating a space and time to explore
the realities of life with stroke in a supportive non-clinical
environment.
The project wants to push the boundaries around health and well-being
of stroke survivors, investigating opportunities to improve quality
of life, enhance recovery and support all those who may need
support. By looking at the real issues for those who have suffered
a stroke the project aims to create a comprehensive stroke service
that targets the cause and lessens the effect.
Services we provide
Rehabilitation Support
Groups - Space and time
in a supported environment to explore stroke
Self Help Group - A group for those who are more independent
and ready to take a more active lead in their own recovery
Joint Working - Collaborating with relevant partners to ensure
quality of service and recognised support for the work we undertake
Signposting and Information - Services for anyone who has had
a stroke (or their carers) and requires assistance or support
with community information or contacts
Health Promotion - Health and well-being information integrated
into services to support people in making necessary lifestyle
changes
Volunteers - Opportunities for individuals to gain experience
in health and social care whilst supporting service users in
regaining their independence
Prevention Work - Intensive support for community / voluntary
groups around stroke awareness and risk factors, with a focus
on black and mniority ethnic communities. Community initiatives
designed to raise awareness of stroke and stroke prevention for
all residents of Hackney
Younger Strokes - A service designed with young stroke survivors
to meet the specialist needs they have identified
How to get our services
In Hackney the service is available to stroke survivors of all
ages
In Camden the project runs one group for the over 50’s
with the support of the Kingsgate Resource Centre
The project accepts referrals from all agencies and can also
accept self-referrals if supported by a GP
