Personal care and support services TLC provides flexible, individually tailored care and support.
We work with people with disabilities, frailty or illness to
ensure that the service is delivered to them in a way that reflects
their right to choose, their individual situations and cultural
needs.
We support children and adults of all ages
who are experiencing a wide range of challenges in daily living.
We work in a variety of venues including the service users’ homes.
Working in different boroughs and being part of the voluntary
sector, TLC has a good understanding of the diversity of the
needs of people with disabilities and can respond in an innovative,
thoughtful way.
All our staff are directly employed and screened by us. All
staff receive appropriate training and supervision throughout
their employment.
Services we provide
Personal Care
Our personal care services include the full range of services
that may be required by people with disabilities. This can
include support with washing, dressing, going to the toilet,
taking medication, cooking or eating. Our staff can support
people with their rehabilitation and can be trained to take
on additional, specialist tasks.
Domestic support
We aim to keep people motivated and in control of their environment,
as well as promoting safety and health. We can provide help with
cleaning, laundry, collecting pensions and paying bills. We believe
that people often benefit from shopping for themselves and can
support them to do so.
Respite care
We understand the stress that caring for a family member can
put on carers, and can provide respite care at home to give carers
a break. We individually tailor this support, carrying out appropriate
activities with the disabled person and allowing the carer useful
time off. This service can be regular or occasional, and can
be during the day or night.
Support outside the home
We have worked in close partnership with several schools to recruit
learning support assistants to work with children with profound
disabilities. Outside school hours we can provide interesting,
fun opportunities for disabled children and young adults to
increase their independence and confidence.
We support disabled adults with normal activities of living outside
the home, such as going to work. We can also support adults to
attend appointments or social events.
Special projects
Our charitable status allows us to seek funding to run innovative
short term projects. These projects may explore new ways of
working and / or strive to meet currently unmet social and
health care needs.
Monitoring
TLC aims to maintain people in their own homes for as long as
they choose to live there. We recognise that this can carry
an element of risk for a variety of reasons. Our staff are
trained to promote independence whilst being alert to risks
and changes in well being, raising the alarm if there is cause
for concern.
How to get our services
Referrals can be made by any purchasing body or by disabled
people or their representatives wishing to buy our services independently.
One of our management team will arrange
to visit to discuss the services required and to carry out
an assessment. Worker(s) will be ‘matched’ to the service user’s
needs, personality and culture. Service users and their carers
are encouraged to keep in regular contact with the management
team who will follow up any difficulties and carry out reviews. |
